Introduction 

Duty of Candour  

St. Philip’s School continues to take steps to carry out our Duty of Candour responsibilities when a serious adverse event happens. We believe in being open with people who experience care, and learning from things that go wrong.  

Purpose 

The purpose of the Duty of Candour provisions is to support the implementation of consistent responses across health and social care providers when there has been an unexpected event or incident that has resulted in death or harm that is not related to the course of the condition for which the person is receiving care. 

 

Report 

This report is in response to legislative requirements that care services and social work services must, by law, produce a short annual report showing the learning from their duty of candour incidents that year, publish it, and notify the Care Inspectorate that it has been published.  St Philips sixth annual report covers the period April 2023 to March 2024 

Within our Duty of Candour, the Governors and Senior Management Team are committed to placing staff, young people, and families at the heart of our services. When harm occurs our focus will be on personal contact with those affected; support and a process of review and action that is meaningful and informed by the principles of learning and continuous improvement.   

 

Summary – 36 Residential Pupils (across all services), 1 Day Pupils – 37 pupils
